History

  • 23 y/o male who had hx. of traumatic right shoulder while in the minor leagues.
  • Tore 70% of anterior labrum repaired arthroscopically in 2001.
  • Re-injured his shoulder recently while diving head first and complains of instability.
  • Complains of  posterior pain with throwing and anterior instability.
  • Physical therapy has not improved his symptoms.

Pertinent Physical Findings

  • ROM 180/170/80, 110/50
  • Internal rotation deficit 30 degrees
  • External rotation on the opposite side is 90 degrees
  • Apprehension test is positive
  • Relocation test is Positive
  • No impingement signs
  • A/C benign
  • Normal scapular-thoracic, rhythm

MRI

  • Disruption of anterior labrum
  • No evidence of RTC tear
  • No Hill-Sachs lesion
  • No convincing evidence of SLAP lesion
